Membership Cancellation - Terms of Service

I understand that Membership cancellations require a minimum of seven (7) days written notice.

understand that my Membership will remain active during the applicable notice period and I may continue attending classes during this time.

understand that any outstanding Membership fees, failed payment fees or account balances remain payable prior to cancellation.

I acknowledge that submission of this form constitutes written notice of cancellation under the Falsegrip Membership Agreement.

Membership Change - Terms of Service

I understand that my Membership will remain active until the requested change date.

I understand that any outstanding Membership fees, failed payment fees or account balances remain payable prior to the change taking effect.

I understand that my 10 Class Pass will commence once my Membership has ended.

Acknowledgement

I acknowledge that submission of this form constitutes written notice of cancellation under the Falsegrip Membership Agreement.
